This session will cover our experience of the development of an Integrated Heart Failure Care model in Leeds. This has brought primary, secondary and community heart failure care services and health professionals closer together and provided enhanced MDT support for PCN and GP employed pharmacists to deliver high quality heart failure reviews for patients in primary care.
