Discharge is a wicked problem which every acute Trust is trying to solve with 'pharmacy' often getting the blame as the last link in the chain when discharge medication is delayed. The Pharmacy Led Safari Discharge project turns discharge prescribing, supply and medication counselling on it's head utilising pharmacist prescribers and physician's associates to create a completely different pathway in which the pharmacy team take ownership for the discharge from start to finish.
