The role of pharmacy professionals in community health services

Come and find out about the exciting role that pharmacy professionals have in Community Health Services. We will also go through some case studies where pharmacy professionals in other sectors have worked alongside Community Health Services teams to provide excellent care to patients in their own homes. The Darzi review highlights the need for more patient care to happen in the community; medicines optimisataion is crucial to support patients at home.

By the end of this session, you will be able to explain the exciting role of pharmacy in community health services and how other sectors can work together for the benefit of our patients.
