Advanced practice HIV – the ART of managing complex co-morbidities
People living with HIV are increasingly affected by age‑related and treatment‑associated comorbidities as life expectancy approaches that of the general population. Chronic kidney disease is a particularly important complication, reflecting a combination of HIV‑related factors, historical antiretroviral exposure, and traditional renal risk factors. This session will provide a concise overview of HIV‑associated comorbidities before focusing on a case of a virologically suppressed individual who commenced antiretroviral therapy in 1990s and is now undergoing work‑up for renal transplantation. The case highlights suspected cumulative renal toxicity from earlier antiretrovirals and the complexities of optimising a modern two‑drug regimen in anticipation of transplant‑related immunosuppression. Through this case, the session will explore the clinical pharmacist’s role in medicines optimisation, managing drug–drug interactions, and supporting multidisciplinary care across HIV and transplant services.
