Enhancing frailty management through nurse-pharmacist partnerships

This session will showcase an innovative nurse–pharmacist frailty care pilot designed to improve outcomes for older adults living with frailty in the community. Delivered in Thurrock, the initiative brought together a frailty nurse and clinical pharmacist to provide comprehensive health and medication reviews, personalised care planning, and safe deprescribing for patients aged 65 and over. Over a 12-week pilot, the multidisciplinary approach demonstrated improvements in medication safety, reduction in anticholinergic burden, and deprescribing of fall-risk increasing medicines, while also helping prevent avoidable hospital admissions. The pilot achieved measurable outcomes including reductions in high-risk medications, financial savings from avoided prescriptions, and improved patient confidence in managing medicines. Shortlisted for four national awards, including the HSJ Patient Safety Awards and Nursing Times Awards, the project highlights how neighbourhood multidisciplinary collaboration can deliver safer, proactive, and more personalised care for older people living with frailty.
