From hospital to high street: how ELOPE is re-engineering CVD prevention through practice based and community pharmacy

This session explores how the ELOPE approach is transforming cardiovascular disease (CVD) prevention by shifting key elements of care into community pharmacies. With CVD remaining a leading cause of preventable illness and death, earlier identification and intervention in everyday healthcare settings are essential. The session will highlight how community pharmacists can improve risk detection, optimise medication management, and support patient engagement closer to where people live. Attendees will learn how the ELOPE model integrates multidisciplinary working, practical service redesign, and community-based delivery to strengthen prevention pathways. By the end of the session, participants will gain insights into scalable strategies that improve access to timely, preventive cardiovascular care.

  • Understand the ELOPE model for cardiovascular disease (CVD) prevention and how it shifts prevention strategies utilising hospital-based experts to practice-based and community pharmacy settings.
  • Recognise the role of multidisciplinary collaboration and utilising community pharmacists in improving early identification, risk management, and medication optimisation for CVD prevention.
