A clinical effectiveness approach to address CVD in Northwest London

Cardiovascular disease (CVD) causes premature mortality and morbidity with significant individual and health system impact. This session will discuss ‘clinical effectiveness’ as a novel approach for addressing CVD, focusing on hypertension. It will showcase work undertaken in North West London with a specific pharmacy workstream utilising population health approaches to improve outcomes, system connection and equity gains to address health inequalities. It highlights exemplar integrated working for place-based patient care.

  • Gain understanding of clinical effectiveness as a model for integrated working driving high quality care
  • Hear about multisystem working and the role of pharmacy to target population health
  • Hear about strengthening integration and place based working
