Neighbourhood health and building integrated care - what does this mean for pharmacy?
Neighbourhood health requires a shift away from siloed models of care and towards integrated, place‑based delivery focused on prevention, equity and outcomes. This session will explore how pharmacy can act as a central neighbourhood asset—moving beyond the traditional labels of “hospital pharmacist” or “PCN pharmacist” to a flexible neighbourhood pharmacist role working seamlessly across settings.
Drawing on real examples from South East London, including integrated community pharmacy services, pharmacist‑led prevention and prescribing, and a multi‑morbidity neighbourhood model of care, the session will demonstrate how pharmacy teams improve access, reduce unwarranted variation and deliver measurable clinical, population and productivity benefits. Attendees will gain practical insight into how integrated pharmacy roles can support neighbourhood care, tackle health inequalities and maximise the value of the pharmacy workforce within modern neighbourhood health systems.
