Pragmatic prescribing to reduce harm for older people with moderate to severe frailty
This session explores how to manage medication reviews in older adults with frailty, including how to discuss benefits and risks of treatment that might not always be in line with national guidance. This will be explored through case studies of older people living with frailty and other co-morbidities. During the session, the deprescribing of inappropriate polypharmacy will be demonstrated incorporating the new British Geriatrics Society's Pragmatic Prescribing.
