Addressing problematic polypharmacy across the systems

The session will cover two polypharmacy projects that are currently taking place in two NHS Trusts in Merseyside to address problematic polypharmacy and associated medicine-related harm. Polypharmacy is becoming increasingly common due to factors including an ageing population and increase in multimorbidity. The National Overprescribing Review highlighted problematic polypharmacy as a significant concern and recognised that medicines optimisation is key to addressing overprescribing. In the presentation we will each discuss the interventions we have made (e.g. medication reviews, advice and guidance sessions), the outcomes and challenges so far and plans for future developments. There will be opportunity for Q&A and discussion at the end of the presentation.
