Structured medication reviews for people with learning disabilities
This session will empower primary care pharmacists to engage in holistic patient-centred medication reviews with patients who have a learning disability, supporting them to improve outcomes from medicines and reduce side effects and over prescribing.
- Understand how to implement reasonable adjustments in practice
- Identify inappropriate prescribing of psychotropics in people with a learning disability
- Use tools to measure and assess behaviours or symptoms that might prompt psychotropic prescribing
- Describe the risks and harms caused by antipsychotic prescribing in this population
- Monitor for side effects from psychotropics effectively
- Take steps to reduce harm from medicines in this population, through either deprescribing or appropriate side effect management
- Carry out a holistic medication review using tailored resources and expert knowledge to optimise medicines.
