The national Discharge Medicines Service is an evidence based, system-wide service that supports patients with medicines as they transition from hospital to home. In this session you will hear about the service, the role of NHS Trusts, community pharmacy and the wider system, and an update on the data. The new national CQUIN will be explained as well as practical tips on how organisations can work together to ensure their patients benefit from the service. NHS Trusts can avoid 1 30-day readmission for every 10 to 23 referrals; join this session to see how you can make NHS DMS work for you, your organisation and your patients.
