Safer prescribing & monitoring of DOACs in Atrial Fibrillation

The significance of calculating CrCl when deciding on DOAC dose in AF has been highlighted many times in recent years, however, we still have issues with getting this right in practice. In Otley PCN, following the MHRA alert June 2020, a service improvement project was designed to improve the initiation and monitoring of DOACs for AF through implementation of a template embedded in the GP IT system. The aim was that by increasing adherence to Amber drug monitoring, this in turn would lead to more accurate dosing and safer prescribing overall.

Learning outcomes:

- The importance of appropriate monitoring of DOACs in AF and the impact that has on correct dosing.
- Greater awareness of how issues around monitoring can be addressed in general practice, potentially through use of a template.
- How implementation of such a template can support achieving the new IIF indicators, particularly SMR-03.
