The pharmacist's role in frailty, deprescribing and diabetes

There are several international guidelines published, which provide frameworks for improving diabetes care in an older population, albeit based predominately on consensus and opinion. The aim of this session will be to discuss emerging concepts in the treatment of diabetes in frail patients and to discuss how best we can tailor treatment for this patient group. Case studies will be used to show how pharmacists can use their skills to improve diabetes care for this population.

• Identify frailty and its impact on diabetes treatment
• Understand the general principles surrounding blood glucose targets and treatment choices in frail older people living with diabetes
• Understand the place of newer anti-hyperglycaemic agents and technology in a frail population
