The session will showcase the NHS England funded Yorkshire & Humber Heart Failure Academy from inception, current state and future plans.

The aim of the Academy is to empower healthcare professionals to better recognise and refer for earlier diagnosis of heart failure. The Academy equips non-HF clinicians with the knowledge and tools that enable better support for patients living with heart failure. 

Various novel, person centred and user friendly educational & developmental resources will be presented.  Initial evaluation will also be shared.
