Lunch Symposium - The PCN Pharmacist: Data Facing or Patient Facing *

The growing clinical pharmacist workforce within primary care networks presents the opportunity to transform traditional ways of working around prescribing and medicines optimisation. A combination of ad hoc reactive patient phone calls alongside proactive prioritised lists of patients present practices with challenges to create a seamless experience for the patient. Too much time is spent on manipulating data to find and process lists of patients and coordinating activities between members of the practice team, reducing the number and quality of patient interactions. Performance indicators and NHSE and Network DES priorities for medicines optimisation need to be translated meaningfully into actionable activities that deliver improved patient care / critical patient medication priorities.
