There are a variety of resources to help with reviewing polypharmacy and deprescribing of medicines. This presentation will cover what is available and introduce the PrescQIPP Improving Medicines and Polypharmacy Appropriateness Clinical Tool (IMPACT) and the supporting data visualisation tool. Sajida Khatri, PrescQIPP Director of Medicines Optimisation and Katie Smith, PrescQIPP Director of Clinical Quality will discuss the clinical evidence included in IMPACT and how to use the IMPACT tools. Interactive case studies will demonstrate how the data visualisation tool can be used to support identifying deprescribing priorities and how they can be managed with IMPACT document, providing deprescribing algorithms and information that can then be discussed with individual patients.
