Managing clozapine in an acute hospital

Mental Health In-Reach has been operating for two years with a mental health pharmacist supporting the acute hospital pharmacy team one day a week. Clozapine was identified as a medicine about which acute pharmacy staff felt most uncertain and particular attention is given to this for in-reach support. Nearly 200 interventions involving clozapine were made between 2022-2024, having a positive impact on patient care. This session will describe the journey for pharmacy staff and patients as we learnt about managing clozapine in the acute hospital setting.
