Aziza Qureshi is a consultant pathway Pharmacist for Frailty and Older People at North East London NHS Foundation Trust (NELFT). She specialises in medicines optimisation, complex polypharmacy, and improving outcomes for people living with frailty. Aziza leads the development and implementation of pharmacy focussed frailty initiative and recently implemented a structured pain review and deprescribing pathways in inpatient rehabilitation wards. She works closely with multidisciplinary teams to embed evidence based practice and enhance patient safety. Aziza is passionate about empowering patients, improving quality of life, and optimising medicines in line with patient goals and the evidence base.
