Jayshree is a multiple award-winning pharmacist and Lead Pharmacist for Frailty and Older People, specialising in medication optimisation for people living with frailty, cognitive impairment, and dementia. Working across East London and Mid Essex, she leads system-wide initiatives to improve medication safety through multidisciplinary collaboration across primary and community care. Her work focuses on preventing avoidable hospital admissions and improving outcomes for older adults through integrated, person-centred care. Jayshree works closely with memory and frailty services and is committed to tackling health inequalities. She is also a Lecturer in Pharmacist Prescribing (Clinical) and is progressing towards Consultant Pharmacist credentialing with the Royal Pharmaceutical Society (RPS).
