Rachel works with the Discharge to Assess team in North Manchester, which is a part of the Manchester and Trafford Local Care Organisation’s Community Medicines Optimisation Service (MLCO/TLCO CMOS). The MLCO/TLCO aim to improve the health of local people in Manchester and Trafford, by bringing together community health services and social care services. CMOS are a team of pharmacists and pharmacy technicians who work holistically and collaboratively with the MLCO/TLCO multidisciplinary teams to provide seamless care across the Primary and Secondary Care interface, reducing medicine related errors during transfer of care and preventing hospital admission/ re-admission.
