The talk will share key lessons on building effective multidisciplinary partnerships, engaging patients in shared decision-making, and scaling neighbourhood models of care. The session will conclude with reflections on how this model can be replicated across integrated care systems to strengthen neighbourhood health and improve outcomes for older people living with frailty.
