    This presentation will describe the design, implementation, and impact of a collaborative nurse–pharmacist frailty pilot delivered within community services in Thurrock. The session will explore how integrating pharmacists into frailty care pathways supports holistic assessments, medication optimisation, and proactive management of complex older adults. Attendees will hear how joint reviews enabled safe deprescribing, reduction in high-risk medicines, and improved patient confidence in medication use, while also delivering financial and environmental benefits through reduced prescribing. The talk will share key lessons on building effective multidisciplinary partnerships, engaging patients in shared decision-making, and scaling neighbourhood models of care. The session will conclude with reflections on how this model can be replicated across integrated care systems to strengthen neighbourhood health and improve outcomes for older people living with frailty.
